Thank you for your interest in the services provided by Phoenix House.
Please complete the enclosed application and forward, along with the following listed below, to Phoenix House Administrator at info@phnxhouse.com
Required Documents for Application:
- Complete Social History
- Current IHH Treatment Plan
Upon Approval and Acceptance at Phoenix House
We will need the following:
- Current Physical
- Clear two step TB Test or Chest X-Ray
- Signed Required Level of Care by a physician
- Current Diet Order
- List of current medications
- Authorization for Habilitation or Intellectual Disability funding (PA) from MCO
Upon receipt of the above items, our leadership team will review your application and contact you.
Important Regulations
Please consider the regulations pertaining to a Residential Care Facility, contained in Ch 57 of the Iowa Code: 135c.23 Express requirements for admission or residence.
A Health Care facility shall not knowingly admit or retain a resident:
- Who is a danger to the resident or other residents.
- Who is in an acute stage of alcoholism, drug addiction.
- The condition or conduct is such that the resident would be unduly disturbing to other residents.
- Who needs a medical procedure, as determined by a physician or services which cannot be or are not being carried out in the facility.
